Pedal fell off!

An odd one yesterday - I rode the tandem with Ms Weasel, dropping her off for aerobics while I went off to buy our dinner for after the return trip.

On the way back to pick her up, on pulling to a stop at lights near Balham, the left hand side stoker's pedal fell off.  On inspection, there is nothing obviously wrong with either the crank or pedal.

Does the pedal need lubricating more maybe?

It may have been turning when the clip hit the ground on each stroke.  Tie them up like I showed you.  Oh - and there's no harm in a bit of grease on the thread - it's probably a bit dry.

I imagine that it is purely and simply that it was not tightened fully.   

A toeclip catching the ground every revolution should make no difference to the tightness of a properly attached pedal as the pedal cage to which the clip is attached simply revolves freely around the axle.

No - it's not toeclips - they are all sitting in the hall as neither of us like toeclips ;)

I'll give them a good tighten tonight.

Is it a proper tandem crankset?  If the stoker's left hand crank was actually a normal right hand crank then precession could make the pedal unscrew.
