I did a surgery teaching session a while ago about fungal nail infections
The key point were
50% of the nails that look fungal aren't, so send clipping to the lab
very little evidence to support topical treatments for fungal nails
clinical cure i.e no more fungus doesn't mean that the nail will look any different
3 months of Terbinafine generally does the job but check liver function after 4-6 weeks
Fungal nails look a bit messy but rarely cause any real issues. Risk of meds Vs benefit needs consideration (especially if you have feet like a Hobbit anyway)
