The dutch have pretty high rates of accidents off of their cycle paths and a high concentration of accidents at the junctions between cycle paths and roads,

Got any link or pointers to data on this?

I haven't but only on hearsay. One of the co-founders of Go By Bike (brilliant company btw) is half-Dutch and he said that Dutch cyclists were far more dangerous on roads  :o
