I've not endo'd one yet. But with an overloaded front bag I found that riding off the tarmac onto loose sand caused the bike to stop dead, saddle lift, back wheel to swing under.

The bars broke when I was climbing out of the saddle and I went over them, standing the bike on its nose as I went. Death to the whole front end.

Were they old M-type bars? Common failure, and there is a brace for them now.

Maybe not endo, but if you slam on the front brakes and get off the saddle to stand up on the pedals, it can get rather interesting....

Get off the pedals too.

My standard dismount procedure is to jump off while hitting the front brake.

The rear wheel has folded under and the bike is landing safely on the rack whilst I'm walking away

If it is any comfort I had my rear wheel drop into a grid cover with the slots placed facing in the same direction as the traffic.

Bike stopped - I continued forwards, striking my goolies on the stem.

I think an "endo" was originally a deliberate "front-wheelie", usually with the brake on, or against a kerb, but some can do them rolling! (I'm basing this on my ~~mis-spent~~ BMX childhood).

Hence they could evolve into flashier stuff like:
