TBF, I can see how, if we are including all the iconography of the railway (and indeed, that was pretty much the brief, according to the nooze), the death/fear of death, particularly of falling off a platform under a train, has been part of the history of the railway since the start. In addition, the scene was a small part of the frieze, reflected in a pair of glasses, indicating with the usual symbolism that it was part of the imagination or interior thoughts of a person, and thus a subjective view.
I still think it was a mistake to include it, though, given the site.
