Sometimes on an entry form, Id get something like P.D.E.Smith (made up any reference toa real person is unintentional) as the name. Running friendly-type events we liked to have first names down even if we didn't refer to people by them. On one ocassion when I asked the first name, I got the answer "Mr."

Back in the early - mid-90s I had a plethora of Organisers called 'Mr.' but after a few years of training them we became less formal.
Still ended up for a couple of years with 3 Orgs. with the same surname of whom 2 had the same forename.
2x4: thick plank; 4x4: 2 of 'em.

Its not Gaz or Gazza, its Gary.

That is a note to the fucking estate agent who started off calling me Mr ....... and within a couple of minutes it was Gaz.  Cheeky fuckpig.

Actually, i'm not concerned to the point of being an arse - unless its a case of being over familiar (Mr Estate Agent).

I'm Gary as well, TBH I'm not keen on my name so I don't find any contractions too irksome, though I'm not a big fan of Gaz.  What does get up my nose is when they spell my name   'Garry'.  I've no idea why.

I don't care if people use Tim or Timothy.  My family call me something else again, which is the fault of my sister when she was younger and couldn't pronounce any of our names correctly (she's Alyson, but universally known in the family as Sassy!)

I've known some Matts and Mandys who insist on being called that, since that's their names, not Matthew or Amanda.
Actually, it is rocket science.

I once wrote a document for work which had as one of its signatories a Tony xxx.
As these are very formal documents he was referred to as "A xxx, <job title>".
When he reviewed it he insisted on a change to "T xxx...".

I have no idea whether Tony is his 'real' name or not!
