There was no payback for doing it - eg a chance of being emailed the report resulting from the survey.

Also completely uncontrolled in terms of population sampled so pretty flawed

Too right!  I wanted to fill it in but CBA to read all those lengthy statements so gave up on the third question. 
Not too keen on the methodology - trying to put words into the respondent's mouth.  I expect they will get a very high abandon rate.

I completed it. It was IMO rather condescending ("We know this is difficult, but... ") but the questions themselves, as opposed to the preambles, were not too long. Uncontrolled though as ian_oli says.
