I've seen a couple of French tandems with quad chainsets.  Because of the chainline it works best with the drive off the front bottom-bracket.  This can be problematical with modern, narrower, more flexible chains, though you can fit guides by the rear BB.

And the DaVinci tandems use a 4-chainring setup, as part of their independent pedaling setup.
