Tick Boxes under Forms Toolbar in Word

I'm trying to produce a document in Word which includes a number of tick boxes that are mutually exclusive, ie only one out of five boxes should be ticked.  I've created boxes, but they can all be ticked.  How can I get them to behave interactively?

you should be using radio buttons rather than check boxes.

I've done this by using the the Control Toolbox, but then I can't protect the document, so that it can be completed in Word; the 'Protect Form' option is greyed out.  Radio buttons don't seem to be available under the Forms toolbar.

I think the only way to do this is to do a macro that checks when one of the boxes is ticked and makes sure the others aren't ticked.
