Yes, it means she lives in a country where the right to an abortion is not enshrined in law. Where even in states where it's available to get to the clinic you have to go through the crowds of protesters. If you have the money, and the ability to get to an abortion clinic in the first place.

Not to mention living in a country where there are people actively trying to ban birth control.

We have no idea about the history of this woman. It's grossly offensive to make judgements about her being a mother at 15.
