Title: Do I just have to suffer with this?

Went to the doctors this morning to have some freezing treatment on sun damaged face. While I was there I talked to him about a pain under my right seat bone causing further pain down my leg. He has diagnosed it as Ischiogluteal Bursitis and said other than Corticosteroid injection there is nothing to do but suffer. Anyone heard of this and other possible remedies?

Bursitis is just a name for an inflamed bursa - a fluid-filled sac that acts as a 'cushion' on pressure points.

One situation where I believe anti-inflammatories actually help healing.

Rest and a change of position should help

Lo-tech relief may result after getting different seats and cushions.
Clenching buttocks may get your muscle to take pressure off painful areas.

Should have said it is only on the bike saddle that it really bothers me, I suppose because my weight is concentrated in those areas.

Suggest getting weight off saddle many times and oft, plus judicious use of Vitamin I.
Try to avoid provoking the pain.

It has been suggested to me that fixed gear is good for this - if you use a highish gear (say, over 75"), then you spend a lot of time out of saddle and when accelerating tend to take all weight on legs.

Depending on your style you will likely spend more time out of the saddle climbing but also likely to descend seated all the way down, and if your'e riding country lanes that's not necessarily a comfortable experience. I was kicked near to death descending the Gospel Pass, a road which gear riders would freewheel down out of the saddle. But there's only one way to tell for sure, and that's give it a go.

I am basically a touring cyclist with my cycling consisting of leisurely riding around 10,000 miles a year of which more than 3,000 will be with a full camping load. At my age, for this type of riding I could not consider using a fixie. Although the problem emanates from the seat bone it causes pains and numbness in my leg which is much more of a problem. Once the pain takes hold there it would be hard to ride by getting out of the saddle. For some of the journey from Mildenhall I was virtually riding with one leg.

Are you sure you don't have a slipped disc as well?

Don't think so. It is rare these days that I get any back pain, I did in my working days but they ended 14 years ago.

I have herniated L4/L5 discs and most of the pain I actually get is down the outside of my left leg.

GP or physio.
Do you get pain if your straight leg is raised passively from 0 - 90 degrees?

No. The only time I get the pain is when sitting on the saddle (tried various ones and all are similar). Firstly I feel tenderness under my seat bones then within a couple of miles I get what I can best describe as a dull ache, down the back of my thigh. If I carry on riding it just turns into a sharp pain and I have difficulty pushing with any weight.

A friend of mine had hip pocket sciatica. No idea if it is anything like your problem.
