Thinking about it a derailleur hanger tends to be a thinnish plate whilst the extenders tend to be quite chunky, so I suspect the 2 purposes are sort of mutually exclusive

The differences between 9/10/11 speed on front chainrings are microscopic to non-existent. The little ring especially so.

Bought and fitted a Sunrace extender (2.25cm / £10); definitely req'd.  Have managed to get the 11-40T (Splumzer ~£20) working on the bike stand.  I think the diameter of the 40t means the chain angle from it to small 32T ring at front is getting close to the limit.  Had to fully adjust and fiddle with F&R mechs.  Will see what it's like on a road test.  Options are try the Deore M591 'MTB' mech, new 11-38T casette, and/or the 30T front ring...  I guess.

ETA.  B-limit screw (original) is almost fully in.  I tried a longer M4 but for some reason it starts moving off the end of the hanger b-limit 'hook'/shelf - don't know why.

I run a 10 speed triple with 9 speed chain and cassette. All works fine.

Update

The 11-40t cassette works fine.  mrs-ao did a local test (inc Bayworth Hill), gear changing smooth, and climbing easier...

With the 48/32 c/s the low ratio is now 0.8; previously 0.94 (with 34t cass).
