Here's my pennyworth...
Most cycling shoes seem like ballet slippers or rock climbing shoes on my broader feet.
Being as they look nice I used to wear them and guess what used to get hot foot on longer rides...
Since switching to wide fit shoes the problem has gone.
SO I reckon it might be a width issue. See "barefoot shoes" and other endless podiatric nonsense...

My issues were definitely the bolts protruding too high above the plate, pushing the footbed up.

I've done a couple of 3hr rides since I swapped bolts and don't have any more problems.
